Mission Statement
The mission of the community health nursing position is to focus on health and wellbeing of the residents of Hanover Township. Services are offered to assist individuals in making decisions to achieve optimal functioning within their environment and to provide choices for our residents to maintain and build a healthier community for all of Hanover Township.

Office Visits
Review and provide information on health precautions and medications as needed.
Screen and monitor vital signs and provide appropriate health information.
Provide information and instruction for dietary modification as needed.
Offer support and discussion as needed and physician referral as appropriate.

Wellness Screenings
Wellness screenings will be held throughout the township at various locations. This screening will include a lipid profile (Cholesterol, HDL, LDL, and triglycerides), anemia, blood glucose, blood pressure, and heart rate. Educational handouts and discussion regarding results will be included. A 12 hour fast is necessary beforehand and registration is required.

Tuberculosis Testing
Mantoux skin testing will be offered by appointment at the nurse's office - 240 S. Illinois Route 59 - Call 630-483-5665 for appointment. Tuberculosis testing is under the guidelines of the Suburban Cook County TB Sanitarium in Des Plaines.

Immunization Clinic
Hanover Township will assist at the Hoffman Estates Immunization Clinic the first Monday of MOST months from 4:00 to 6:00 p.m. at the Hoffman Estates Village Hall, 1900 Hassel Road, Hoffman Estates. For more information call 630-483-5665. This is a free immunization clinic for children ages 2 months-18 years old. There will be a $5.00 supply fee per injection charged, bring your child's immunization records with you each time to receive service.

Home Visits
The nurse will do home visits and provide services to the homebound residents who do not qualify for these services under any other program. There is no charge for this service. A referral can be taken by Doctor's, nurses, hospital discharge planners, residents, etc. A needs assessment will be done on initiation and referrals to other community/social services will be provided.
- Heart & Lung sounds
- Assessment of catheters, feeding tubes, oxygen equipment
- Instruction to patient and/or caregiver in activities of daily living.
- Dressing Changes
- Care and instruction in prevention of decubitis ulcers
- Administration of IM or Sub-Q medications as ordered by resident's physicians.
- Assist with filling insulin syringes as needed
- Provide instruction to patient and/or caregiver in administration of medications.
- Provide assistance with oral medication compliance (assist in indigent drug programs as appropriate).
- Blood sugar monitoring.

Lending Closet
The Lending closet provides medical equipment to Hanover Township residents. Bath seats, canes, commodes, crutches, transfer benches, wheel chairs are some of the available items. A security deposit is requested and refunded when the equipment is returned. The Lending closet is located and operated by the Senior Center, for more information please call 630-483-5600.
